A 56 y/o patient presented with an extremely atrophic edge of 1mm in the anterior sector of the mandible. In this case, bone grafts with membranes or therapy with individual Linkow sheets would be indicated. We were able to insert 3.75 mm threaded implants with external hexes. We started by opening the flap, then made several perforations in the ridge using a fissure bur (tungsten), joining them to prevent ridge fracture, the same procedure as for sheet placement.   We continued by sequentially inserting the bone expanders
1-Expander 1 Art. no 1025: Expansion sheeting 1
2- Expander 2 Art. no 1830: Expansion sheeting 2
3-Expander 3 Art. no. 2938: For implants of diameter 3 to 3.30 mm.
4-Expander 4 Art.no.3345: For implants of diameter 3.8 to 4.20 mm.

Once the expansion protocol had been eliminated, the bleeding was profuse and heat-less osteotomy was ensured. There was perfect control of the instrument. No fractures or perforations of the matrix occurred. Four threaded 3.85 mm. implants with external hexes werw inserted, although the case was resolved satisfactorily for the patient. Currently we would use implants of diameter 3.30 mm, with internal hexagonal heads, which would have an emergency profile in the crown of approximately 3.60, with the aesthetic results being more acceptable for the professional.
